What To Bring To Your First Visit

  • Prescription for physical therapy (if you have one)
  • Insurance card(s)
  • Photo ID (e.g., your driver’s license)
  • Any diagnostic reports including X-ray/MRI reports

To save you time and effort, please print and fill out this Patient Information form and bring it with you to your first visit. This will ensure that your appointment time can be best spent on your direct patient care. If desired, you may fax your forms in to (712) 215-7393 at least one day before your scheduled visit.
